Patient preferences for end-of-life care are an integral component of inter professional, patient-centered care.  Over the past 25 years, researchers have investigated patient and provider attitudes about end-of-life care and the importance and challenges of advance care planning (ACP).  Federal and state legislation has resulted in formal, legal advance-directive documents.  Professional organizations and advocacy groups have focused on education, encouragement, and support for families and healthcare providers about ACP and the importance of discussing patient preferences before the last stages of life, when critical decisions can be difficult.  State-specific documents, provider education, and resource training tools will be included.

This webinar will address how to describe ACP, why it is used, and triggers for initiating ACP discussion, while considering patient wishes, preferences, and beliefs.  It will also review the background of ACP, highlight key issues, and offer strategies for planning and implementing a successful program.

Responding to the Opioid Epidemic (Three-part Webinar Series)

In 2018, APA’s Planning and Community Health Center and Healthy Communities Collaborative is hosting a series of webinars to explore the intersection of planning and the opioid epidemic.  Designed to share local experiences and national resources, the series is also meant to launch a network of members and peer professionals working to understand the crisis and our roles in it.

The second session, Responding to the Opioid Epidemic, will look at how communities can respond to the opioid crisis. Best or emerging practices and community experiences will be shared. Dr. Brandon Marshall from the Brown University School of Public Health will share his experiences with Prevent Overdose RI, Rhode Island’s drug overdose surveillance and information dashboard. Dr. Robert Park from the East Tennessee State University will share his experiences in opening a non-residential opioid treatment center and the challenges involved.

ESRD QIP: Final Rule for CY 2018 Call
