Course Description

Medication reconciliation is a critical task in healthcare settings. With a focus on avoiding medication errors such as omissions, duplications, dosing errors, and evaluation of drug interactions, medication reconciliation processes are a key factor contributing toward patient safety.

In this webinar, consultant pharmacist Sunae B. Tom will share the five steps of the medication reconciliation process, review regulatory guidance and discuss quality initiatives related to medication reconciliation. She'll guide us through a closer look at the impact medication reconciliation has related to the transition of care between a hospital and long-term care setting, and review several case studies.

The webinar will be recorded and made available on the HAH website for on-demand viewing within one week of the live webinar date.

Sunae B. Tom, Pharm.D.
Pharmacare

Sunae B. Tom, Pharm.D., is a graduate of the University of California, San Francisco School of Pharmacy. With a strong background in geriatric care, medication regimen review, chronic disease management and CMS regulations for skilled nursing facilities, she also has experience with inpatient, home infusion therapies, specialty medications, and oncology services.

As a consultant pharmacist serving long-term care facilities for 16 years, Sunae thrives on collaborating with a multidisciplinary team environment at skilled nursing facilities to provide exceptional pharmaceutical consulting services and developing, implementing and auditing programs and initiatives to comply with federal and state regulations.
