Join us as we explore the Age-Friendly Health Systems (AFHS) model which focuses on delivering high quality, person-centered care through the 4Ms: Mentation, What Matters, Mobility and Medication. In these training sessions, we will examine evidence-based practices that positively impact our kupuna’s quality of life. Complete each training session in this free series to earn a continuing education certificate.
Participants in this module will:
Examine how dementia affects the brain and the senses
Recognize the importance of screening for depression among residents in long-term care
Describe the difference between POA for financial matters vs. health-related matters
Explore social determinants of health and the cost of social inequities
Identify risk factors for falls
Discuss how medications can be incorporated into an age-friendly health plan
Recognize the risks associated with taking pain medications and opioids
Target Audience
Healthcare personnel in the medical and dental fields and individuals who care for seniors.

Completion of all eight (8) videos in this module has been approved for 2.5 contact hours for nurses and social workers. To earn a continuing education certificate, individuals must register for the AFHS module, complete each training session in the AFHS module, and complete the HAH evaluation for the AFHS module.
